Tizanidine
Tizanidine is technically a muscle relaxant that has been around for a while now. It usually comes as a generic medication. Tizanidine activates Alpha2 auto-receptors. These receptors essentially sample the amount of adrenaline released by a given neuron and turn down the release when it determines there is enough. We can fool the neuron into thinking it has released “enough” adrenaline by activating the receptor directly. It is the neuron’s way of determining what it has done and tizanidine tells the neuron to stop releasing.
In repeated off-label studies Tizanidine has been shown to be helpful in treating fibromyalgia. While it is not FDA approved for insomnia, alpha2 activation is a well-known mechanism for Growth Hormone release, a process that generally occurs during deep sleep. Deep sleep and growth hormone release generally go hand in hand. The improvements seen using tizanidine in studies of fibromyalgia may stem from the direct improvement in sleep architecture, relief of pain through muscle relaxation and through growth hormone release. Not all muscle relaxants improve fibromyalgia, so there is likely something involving these mechanisms that is involved.
Directions
Unless directed otherwise:
- Start with one of 2 mg pill at bedtime
- If you aren’t experiencing sleep improvement, increase your dose to 4 mg.
- Expect sleep improvement but not a knockout pill.
- Side effects can include some mild early morning grogginess and difficulty starting the day but it is usually well tolerated.
If you wake in the middle of the night and cannot fall back asleep keep increasing the dose as detailed above. If this continues after even 4 mg, add a second agent that we may have talked about (e.g., baclofen, tiagabine), or even a third agent if this is already your second agent. Remember the idea of a sleep equalizer; we are using different but complimentary mechanisms to facilitate sleep quality.
